CareThe Commonwealth Care Health Insurance Program is a program run
by the Commonwealth Health Insurance Connector. This program connects eligible
Massachusetts residents with approved health insurance plans and helps them pay
for it. Commonwealth Care pays the total cost of health insurance for qualified
individuals who have income at or below the federal poverty level and helps pay
toward the cost of insurance for other qualified individuals. HealthFirst's Financial Counselors Paulette Couture and Helen
Teixeira are available to answer your questions or help you apply for insurance
programs and whether you are a HealthFirst patient,
interesting in becoming one, or a patient of another physician. They can be
reached at (508) 679-8111 Ext. 238 or 361 or by stopping in to see them.
- Project
Bread SNAP Program (Food Stamps) HealthFirst's
Financial Counselors will also assist any Greater Fall River resident in
applying for this program, regardless of where you receive your health care.
- Care Coordination Program - This program, designed to
provide you with extra support for your health, is offered to Massachusetts
residents between the ages of 40 and 65 who are insured or uninsured and meet
certain income criteria. The Care Coordination Program is an initiative to make
sure patients get access to the health care they need. Patients who participate
in this program gain access to a personal health care team who can help you
sign up for health insurance, provide information about medical services that
might be right for you and help schedule important tests such as colon cancer
screenings, pap tests and mammography screening for women and prostate
screening for men.
- Homeless
Connection - Through a grant generously provided by the Blue Cross
Foundation, HealthFirst's Nurse Practitioner Julie
Hetherington visits local homeless shelters, food pantries, soup kitchens and
other agencies to extend a kind and helping hand to those who might not
otherwise seek medical care. Connecting with the homeless on a one-on-one basis
establishes a level of trust, care and concern ultimately leading to a visit
with a health care provider and the establishment of a "health care home".
