Healthy Smiles Dental Program

Our experienced Healthy Smiles team includes Massachusetts-licensed dentists, hygienists, dental assistants, and an outreach coordinator. Our Outreach services address the need for increased access to quality dental health care for children in Fall River Public Schools, pre-schools, and other outreach programs. Families will receive notification of these services through your child’s school or other outreach program once or twice yearly. Families that opt to participate will receive a letter with their child on the day of the appointment informing them of the services provided. If future treatment is necessary, you will receive a follow-up letter noting the treatment that is being recommended for your child. It is recommended that everyone visit a dentist twice a year for a routine exam and cleaning.

All children can take part in this program. If you want your child or children to take part, please fill out this consent form.
If you have more than one child, please fill out this form for each child you want treated by the Healthy Smiles team.
 

Please note: These services will be provided at school during school hours. On the day of the visit, your child will bring home a letter with the services that were done. If your child needs more treatment, you will get a letter with information about the additional treatments that are recommended for your child.
