2025 Community Health Needs Assessment

The 2025 SoCHA Community Health Needs Assessment
(CHNA) represents a collaborative community-wide approach
that incorporates a comprehensive analysis of health
data, stakeholder interviews, community focus groups and a
community survey to identify and address the most pressing

2022 Community Health Needs Assessment

HealthFirst Family Care Center (HealthFirst) and the Family Healthcare Center at Stanley Street Treatment & Resources (SSTAR) conduct a triennial Community Health Needs Assessment (CHNA) to examine the health and support service needs of Fall River residents to develop sustainable primary care and support services that are essential to the community. Input from health center patients, community organizations, and residents was used in the implementation of this assessment. It will serve as a reference document for HealthFirst and SSTAR in properly addressing the health needs and barriers to care faced by the community health centers. Read more…
