The Care Management Program at HealthFirst Family Care Center, Inc. applies systems, science, and information to assist patients and their support systems to become engaged in a collaborative process designed to manage medical, social and mental health conditions more effectively. Our goal is to help patients achieve an optimal level of wellness and improve coordination of care while providing cost effective, non-duplicate services. The underlying premise of our Care Management Program is based on the fact that when an individual reaches the optimal level of wellness and functional capability, everyone benefits-the individuals being served, the health care systems, as well as reimbursement sources.
Care management services are most effective in a climate that allows direct communication between the care manager, the patient, the primary care provider, and appropriate support personnel. Our team is comprised of a Registered Nurse, a Registered Dietitian, Certified Diabetes Educators, Interpreters, a Smoking Cessation Specialist, and a Licensed Social Worker. Referrals made to our Care Management Program are tailored to meet the individual patient’s needs, respecting the role of the patient to be the decision maker in their care planning process.
Your referrals for diabetes management, and nutrition counseling are welcome.
Individuals do not need to be HealthFirst patients to utilize these programs.
Individual and group appointments are designed to meet the educational and cultural needs of our patients.
Our Diabetes Education program has been accredited by the American Association of Diabetes Educators since 2012.
